Instructions (READ THIS FIRST)
If you have an accounting question or issue that has come to your attention, please submit the following for in order for accounting to take care of your request.
It is imperative that you fill out AS MUCH DETAIL as you can! You can never ask too many questions. This will ensure that accounting has everything they need to resolve the issue.
Please let the client know that you are submitting a trouble ticket to accounting and that you will keep them updated on it's progress.
Once you submit this form, a trouble ticket will be added into accounting and YOU will be the manager of that ticket. If accounting has any questions, you will be responsible to get the information from the client and update the trouble ticket.
